How do I get my Voucher?

Request a Voucher

There are a few ways you can request a Voucher:

A) You can request your voucher online –>

B) Download the Voucher Request form as a word doc or pdf and fill it out.

OR

C) Pick up a copy of the form at:

3 – 5 pm the first Saturday of every month

OR

D) Call (877) 729-7475 or email PawsVaccineClinic@gmail.com to request a voucher and we will contact you for your information.

Submit the Voucher Request form

If you did not fill out the request form online and instead downloaded or picked up a form, you can mail the Voucher Request form back to us at:

or scan and email it to us at PawsVaccineClinic@gmail.com

Next the Voucher will be sent to you

PAWS will then mail a voucher to the name and address supplied on the request form.

One voucher per household.

Make a Spay/Neuter Appointment

Make an appointment for a spay (female) or neuter (male) surgery with a veterinarian on our “Participating Vets” list on the back, or ask your veterinarian if he or she will accept the voucher.

Give the Voucher to the Vet

On the day of surgery, give the voucher to your veterinarian to complete.

The Vet will Submit the Voucher and invoice

Your veterinarian will then submit the voucher, along with an invoice showing the total cost of the surgery, to PAWS for SJACS.
